On 12-1-2011 Smile Designs at Agape Family Dentistry, Dr. Charlotte Gerry DMD PA was burglarized. Computers were stolen with Date of Birth, Social Security Numbers, and Drivers license information, and other pertinent information used for medical billing. It is suggested that you contact credit bureaus, banks, or credit monitoring services. The Jacksonville Sheriffs office is investigating.
